HISTORY OF PRESENT ILLNESS: unhurried was diagnosed with Cystic Fibrosis at stick out. This disease has ca apply for unhurried to have recital of gallstones that have been found and removed through ERCP 5 days ago. When the ERCP was administered, the physician also found the patient to have a pancreatic pseudocyst. Treatment for the pseudocyst overwhelmd an endoscopic-assisted drainage using an endoscope. The patient was also diagnosed with pancreas divisum at birth. Patient has had no problems or symptoms with this birth defect.
Patient was diagnosed 8 years ago with hypertension (high seam pressure) and has been taking Furosemide (or Lasix) since the diagnosis. She has also been prescribed Azathioprine for being diagnosed with rheumatoid arthritis 2 years ago. Patient also has a history of very high triglyceride levels and has been diagnosed with hypertriglyceridemia.
Patient also has a history of lancinate pancreatitis.
Patient may have a history of alcohol abuse.
SYMPTOMS: Patient came in with severe type AB pain, nausea and vomiting, along with feeling fatigued.
PHYSICAL EXAMINATION FINDINGS: Because of the patient being obese, we tested and found idealistic levels of lipids and too lots calcium in the blood to diagnose her to have penetrative hyperlipidemia along with having modest hypercalcemia. Because hypercalcemia, hypertriglyceridemia, cystic fibrosis and history of acute pancreatitis, a blood test was done to check the insulin levels. We did gravel a decreased insulin level and blood amylase levels were slightly elevated parallel with lipase concentrations. This led us to believe the patient is experiencing these symptoms because of inveterate pancreatitis.
A stethoscope was used to listen to the abdomen and the physician remark no bowel sounds. An abdominal CT scan was used to diagnose the patient with chronic pancreatitis.
